You will trigger an automated email response confirming your request. If you do not receive an email verification please ensure that you check your email spam folder.

    PERMISSION FORM FOR THE DIRECTORY & EMAILS

    For privacy considerations, we ask that you tick the appropriate box as acceptance to appear (or not appear) in the directory and to receive (or not) village emails by checking one or both of the two boxes.

    If you do not want to be included in the directory and do not want to receive e-mails, please complete only the SECTION 1 of the form. If you want to be included in the directory and to receive e-mails (or if you choose to only participate in one of the two options) please complete all SECTIONS 1 & 2 of the form.

    We also request that if you change your email, your address within the village or telephone numbers, that you contact the management office with the amended information.

    ADDITIONAL INFORMATION

    The e-mails that you will occasionally receive include information alerting residents to:

      1. Messages from the Village Amenities Committee (VAC) that manages the common elements within the Village
      2. Messages from your Swan Lake Condominium Board
      3. Messages from the Property Management Company as agents of VAC
      4. Messages sent to all, or a subset, of the village alerting them of maintenance issues that may affect them
      5. Messages alerting residents to Swan Lake Village social events and activities

    Please note: your permission to receive Village e-broadcasts on this form does not constitute permission to receive legal documents electronically from your Condominium Corporation. Please contact your Condominium Corporation Management Office for the requisite government forms.           

    New directories are produced every year or every two years depending upon the number of changes that arise over the course of a year. You may withdraw your permission any time before a new directory is produced or unsubscribe at any time, if you decide to receive no further e-mail messages. Once you are in possession of a new directory insert either shred the old insert, or if you have no shredder turn it in to the PMO or to the distributor when you collect your new insert.

    REMEMBER: SECURITY IS EVERYONE’S RESPONSIBILITY.

    E-mail addresses are kept confidential (we never provide them to any third party) and messages are distributed in a way that does not expose residents’ e-mail addresses. (Please never give your directory to anyone outside of Swan lake. (“Hacking” can be a significant problem.)
